Group Class Application

Filling out this form is an application for a spot in the class, the spots are first come first serve. We will contact you to confirm the available spot and discuss payment. No payment is required to apply for the class, there is a $50 non-refundable deposit required to confirm your spot in the class. A remaining balance of $160 will be due on the first day of class.
